Mrs Rachael Baker – Practice Manager
Based at Pontyclun Surgery, Rachael is responsible for the day to day running of the practice and can help you with any administrative or non-medical aspects of your health. Rachael also deals with problems and complaints you may have to do with the Practice and would welcome any ideas you have that you feel would improve the services we provide.
Mrs Joanne Davies – Assistant Practice Manager
Joanne undertakes administrative work for the practice and will deputise in certain areas for Mrs Rachael Baker, Practice Manager when she is on annual leave.
Sister Rhian Lang Advanced Nurse Practitioner (not in the Practice on a Wednesday)
Rhian has a BSc in Community Health Studies and has a title of ‘Specialist Practitioner – GP Services’. Rhian works in Pontyclun and Llanharry and has an interest in Respiratory problems (Asthma & COPD). Rhian is also a supplementary Prescriber.
Rhian will help with the on-call surgeries and run her own clinics for patients aged 7 and above
Sister Lauren Griffiths – Specialist Practitioner – General Practice Nurse & Practice Nurse Manager (not in the Practice on a Wednesday) – Currently on maternity leave
Lauren will work in both Pontyclun and Llanharry Surgery and has a special interest in Family Planning and Well Woman, Asthma, COPD and Diabetes. Lauren is also a Dementia Champion.
Sister Mandy McMurray – Practice Nurse (Wednesday, Thursday and Friday only)
Mandy works in both Pontyclun and Llanharry surgery and has a special interest in Diabetes, Epilepsy, Contraception and Sexual Health and Cancer Care
Sister Jenna Williams – Practice Nurse (not in the Practice on a Wednesday)
Jenna joined the Practice in October 2018 and works in both Pontyclun and Llanharry Surgery.
Sister Melissa Evans – Practice Nurse (Monday, Tuesday and Wednesday)
Melissa joined the Practice on 2nd January 2018 and will work in both Pontyclun and Llanharry Surgery. As Melissa has previously worked in a hospital setting she will be able to transfer some of her skills into general practice but will need training in other areas such as cervical cytology, diabetes and asthma management.
Mrs Natalie Williams – Healthcare Assistant
(Monday, Tuesday and Wednesday only)
Natalie is responsible carrying out Chronic Disease monitoring(Diabetes, Heart Disease, Stroke etc). Natalie also runs clinics for New Patients Screenings, Blood Pressure checks, Blood tests, Wound checks, Vitamin B12 injections.
Eleanor Hucker & Joanna New – Practice Secretaries
The secretarial team are responsible for typing all hospital referral letters and dealing with general queries in relation to hospital appointments, scanning incoming documentation from the hospital, clinical coding medical records.
Jenna Kelly-Taylor and Cloe Jones. We have a dedicated team for processing acute and repeat prescription requests. More information regarding our prescription policy can be found on this website.
Christine Howells, Kimberly Hughes, Victoria Done, Dawn Coombes, Kay Crooks, Sophie Williams, Julie Jones, Georgia Clissold-Gough & Catrin Maunder
We have a dedicated team of reception staff who are your first point of contact with the surgery. Please remember that they have a job which is often difficult and are not able to please all of the people all of the time. They do act under instructions so if you are unhappy do not take it out on them.
Christine Howells & Victoria Done are also qualified Phlebotomists.
The Practice has a Health Visitor Team based at the surgery. They can be contacted on 01443 228 364. The Health Visitors care for children aged less than 5 years. They operate our Well Baby Clinic and carry out developmental checks. The Health Visitors work from Pontyclun and Llanharry Surgeries.
The Practice has a District Nursing Team who provide nursing care for patients at home, who are unable or too ill to attend surgery. The team can be contacted on 01443 443443. Please note that the District Nursing team is no longer based at the Practice
